Enter current values. Use account statements for financial assets and a realistic market value for property and vehicles.

Personal financial statement: questions people ask
Is this the SBA's Form 413?
No. This is an original general-purpose format. If an SBA lender asks for SBA Form 413, use the official form from sba.gov.
What is a contingent liability?
A debt you could become responsible for, such as a loan you co-signed or personally guaranteed, or a pending lawsuit.
Should retirement accounts be included?
Yes, at current value. Some lenders discount them because of withdrawal penalties, so list them on their own line.
How should I send this securely?
Use Protect PDF to add a password, then share the password by phone or text, not in the same email.
Is it a problem if net worth is negative?
It is a fact the reader will weigh. Do not leave out debts to improve it; knowingly false statements to a lender can be a federal crime.
