Permission Slip
For teachers, coaches and youth leaders who need a quick yes or no from parents for an on-site activity, such as a class movie, guest speaker, special event or club meeting. For off-site trips with transportation, use the field trip consent form.

Permission Slip: questions people ask
When should I use this instead of the field trip consent form?
Use this short slip for activities at school or your usual venue. Off-site trips with transportation, meals or overnight stays need the field trip form, which collects medical and emergency details.
Can parents sign electronically?
Usually yes, if your school or organization accepts e-signatures. Send the slip with Request signatures or publish it as a web form.
What if a parent says no?
Plan a supervised alternative activity and make sure the student isn't penalized.
How long should we keep signed slips?
Follow your district or organization's records policy. Many keep them at least through the end of the school year.
