Parental Consent Form: questions people ask
Does a parental consent form need to be notarized?
Usually not, but hospitals, airlines or schools may ask for a notarized copy, and notarization makes the signature harder to dispute.
Is this the same as giving someone custody or guardianship?
No. It is a limited, temporary authorization. Longer-term arrangements may need a state power of attorney for a minor or a court order.
Will a hospital accept it?
Hospitals usually treat emergencies regardless. For non-emergency care, policies differ; including insurance details and a doctor's contact helps.
Do both parents need to sign?
One parent's signature is often enough, but a custody order or the receiving organization may require both.
