Field Trip Consent Form: questions people ask
Does this form protect the school from liability?
It documents consent and medical information; it is not a full liability waiver. Whether schools may ask for waivers, and whether they are enforceable, depends on the state and district.
Who can give medication on a trip?
Most districts require a separate medication authorization signed by a parent and sometimes a doctor, and only trained staff may give it.
Can parents sign electronically?
Generally yes if the school accepts e-signatures. Bring printed copies on the trip in case phones lose signal.
How is this different from the permission slip?
The permission slip is a short yes/no for on-site activities. This form adds transportation, medical, emergency and insurance details needed off site.
