Incident report: questions people ask
Does this replace OSHA Form 301?
Not necessarily. OSHA accepts an equivalent form if it contains the same information, but employers covered by recordkeeping rules must still maintain the OSHA 300 log. Check OSHA's requirements; this is not legal advice.
When must serious injuries be reported to OSHA?
Under federal rules, a work-related fatality must be reported within 8 hours, and an inpatient hospitalization, amputation or loss of an eye within 24 hours. State plans may differ.
Should near misses be reported?
Yes. Near misses show hazards before someone is hurt, and tracking them is one of the most useful safety practices.
Who should see completed reports?
Limit access to people who need it. Injury reports contain personal and medical information; store them separately from general files.
