What it is
What Form DS-5504 is for
Form DS-5504 lets you get a new passport by mail, usually at no cost, if your passport was issued less than a year ago and you've since changed your name, if it has a data or printing error, or if it was a limited-validity passport issued at the full fee. Unlike DS-11, you sign it yourself and mail it with your current passport.

Form DS-5504: questions people ask
Is there a fee?
No fee for routine service. You pay only if you request expedited service for a name change or limited passport replacement.
My passport is more than a year old. Can I use DS-5504 for a name change?
No. The name-change option requires both the name change and the passport issue date to be less than a year ago. Otherwise use DS-82 or DS-11.
Can I use DS-5504 for a lost passport?
No. A lost, stolen or damaged passport must be replaced in person with DS-11.
Can I sign it electronically?
The form is printed and mailed. Sign the printed page by hand.
Will my new passport have the same number?
No. Each newly issued passport book or card gets a different number.
