What it is

What Form 1040-V is for

Form 1040-V is the voucher you mail with a check or money order for the “Amount you owe” on Form 1040, 1040-SR or 1040-NR. You don't need it if you pay online. The PDF on irs.gov is the 2025 edition, for 2025 balances; use the 2026 voucher, usually posted in December or January, for tax due with a 2026 return. Payment is due April 15, 2027, even if you get an extension to file.

Line by line

How to fill out Form 1040-V

  1. 1

    Line 1 — your SSN

Enter your SSN, or for a joint return the SSN shown first on the return. Use an ITIN if that's what you have.

  2. 2

    Line 2 — spouse's SSN

For a joint return, enter the SSN shown second on the return.

  3. 3

    Line 3 — amount

Enter the amount you're paying by check or money order. If you pay online, don't use the voucher at all.

  4. 4

Line 4 — name and address

Print your name(s) and address exactly as shown on your return.

  5. 5

    Prepare the check

Make it payable to “United States Treasury”, write your daytime phone, SSN and “2026 Form 1040” (or 1040-SR or 1040-NR) on it, and write the amount as $ XXX.XX without dashes or lines.

  6. 6

    Mail it loose

Don't staple or attach the check or voucher to the return or to each other. Put them loose in the envelope.

Common Form 1040-V mistakes

  • Stapling or clipping the check and voucher to the return.
  • Making the check payable to “IRS” instead of “United States Treasury”, or leaving off the SSN and tax year.
  • Using last year's voucher or the wrong year on the check.
  • Mailing a voucher for a payment you already made online.

Where to send it

Mail the voucher and check to the payment address on page 2 of Form 1040-V for your state (https://www.irs.gov/pub/irs-pdf/f1040v.pdf); if you're filing a paper return, the voucher goes in the same envelope. The voucher has no signature line, but a paper return still needs a handwritten signature. Paying online at https://www.irs.gov/payments (Direct Pay, your IRS online account, EFTPS or card) replaces the voucher.

Filing rules change. Confirm the address or portal on irs.gov before you send.

Form 1040-V: questions people ask

Which year is this Form 1040-V for?

The PDF on irs.gov today is the 2025 voucher, for balances on 2025 returns. For tax due with a 2026 return, use the 2026 voucher once it's posted, usually in December or January.

Do I need Form 1040-V if I pay online?

No. The voucher is only for payments by check or money order. Online payments through Direct Pay, your IRS account, EFTPS or a card don't need it.

Can I pay in cash?

Yes, through retail partners after registering at the IRS's official payment provider. The 2025 voucher says the limit is $1,000 per day per transaction.

Does an extension give me more time to pay?

No. An extension (Form 4868) gives more time to file only. Interest runs on tax not paid by April 15.

Can I fill in the voucher here and print it?

Yes. Fill in the IRS PDF in PDFBetter without an account, then download it with a free account and print it. PDFBetter doesn't send payments or returns to the IRS.
