What it is

What Form 4506-T is for

Form 4506-T requests free IRS transcripts — summaries of your tax return, tax account, wage and income documents, or proof that you didn't file. Lenders, schools and agencies often ask for transcripts to verify income. The IRS's online Get Transcript tool is usually faster; the form is for mail or fax requests.

Where to send it

Mail or fax it to the address or fax number in the charts on page 2 of the form, based on the state you lived in (or the business was in) when the return was filed; there are separate charts for individual and other transcripts. The IRS must receive it within 120 days of the signature date. Transcripts are mailed to the taxpayer's address on record — the IRS no longer mails them to third parties. Filing rules change. Confirm the address or portal on irs.gov before you send.

Form 4506-T: questions people ask

Is there a fee for transcripts?

No. Transcripts are free. A full copy of a return requires Form 4506 and a fee.

Can a lender get my transcript directly?

Lenders typically use a separate income-verification process with their own form. With Form 4506-T, the IRS sends transcripts to the taxpayer, not to third parties.

Which transcript do I need?

A return transcript shows most line items, including AGI, and is what lenders and schools usually want. An account transcript shows payments, penalties and adjustments.

Why was my request rejected?

Common reasons: the form arrived more than 120 days after signing, lines were left blank, or the name and address don't match IRS records.

Can I sign it electronically?

The form doesn't describe an electronic-signature option for mail or fax. Print it and sign by hand unless the IRS tells you otherwise, or use Get Transcript online instead.
